VETERAN
APPLICATION
Lake County Honor Flight honors American veterans for their service and sacrifices by flying them to Washington, D.C., to visit their respective memorials at no cost to them. Priority is given to the oldest veterans and those in hospice care. After that, we process applications from veterans of the Korean War, Vietnam War, and Cold War (served through 1975). Unfortunately, because we have an extensive waiting list of older veterans from these periods, we’re unable at this time to accept applications from those who served more recently.
Please complete the online application below, or click here for a printed version (PDF) that can be mailed. If you have already submitted an online application but need to provide updated information, please go to the contact form at this link and include details in the Message field. Please do not resubmit this form.
Lake County Honor Flight strives to create the best experience on the Honor Flight mission for our veterans, so we assign guardians to travel with each veteran. If you would like a specific family member or friend to accompany you as your guardian, please enter their information in the form below. Guardians must be 18-74 years of age, in good health, and approved by the Lake County Honor Flight Trip Director. Spouses, domestic partners, or significant others are not eligible to serve as guardians. At the time of flight assignment, guardians must submit a guardian donation and attend a mandatory guardian training course.
Thank you for your service to our country!
