APPLICATION

Please complete the digital application below. This application is to request consideration to serve as an Honor Flight guardian. Once assigned to a veteran, you will be contacted. If you have already submitted an online application but need to provide updated information, please go to the contact form at this link and include details in the Message field. Please do not resubmit this form.

 

Important:

  • Donation: As a guardian, you must agree to donate $1,400.00 as determined by the Lake County Honor Flight Board of Directors. This donation is due within 5 days of being assigned to your flight. Your donation is tax-deductible. 

  • Age Limit: Guardians must be under 73 years old.

  • Health and Approval: Guardians must be in good health and approved by the Lake County Honor Flight Trip Director.

  • Eligibility: Spouses, domestic partners, or significant others of veterans are not eligible to serve as their assigned guardians.

  • Training: Once assigned to a mission, you must attend a mandatory guardian training course. Currently, only in-person training is available, and it is conducted on a specific date. Please note that makeup dates for the training are not possible.

  • If you are applying to be the guardian of a specific veteran, you must be at least 15 years younger and one generation removed from that veteran.

  • Lake County Honor Flight reserves the right to request reference(s) from any guardian applying to participate in the program.
